I just bought two American Express Gift Cards -

One in the amount for $50.00 and the other one in the
amount of $100.00. There was a advertisement for the Spring Fling Gift Promotion. But the CVS in the Linwood Shopping Center,Fort Lee did not have any forms in store. I ask store manager, said he was not informed. Please send a form to me.

How do you know there isn’t a $3.95 activation fee on the 25 Spring Fling reward card also?
